The standard BMI formula is the same for men and women, but body composition differs significantly:

- Ethnicity: Asian populations face higher metabolic risk at lower BMI thresholds; some guidelines recommend 23.0 as the overweight cutoff for Asian adults
- Height extremes: BMI overestimates fatness in tall people and underestimates it in short people
